We are looking for a highly motivated person to deliver a culturally appropriate and sustainable service for Aboriginal people with chronic health conditions.
Duties include supporting those with chronic conditions, preparation of care plans, ensuring access to health services, arranging referrals, liaison with community-based services in order to connect community members, reporting and statistical analysis.
