About the Role
The Tasmanian Aboriginal Centre is seeking a committed Culture, Care, Connect Worker to join our Aboriginal Health Service team in Nipaluna/Hobart.
This role provides culturally safe aftercare support to Aboriginal and Torres Strait Islander individuals, families, and community members following a suicide attempt or suicidal crisis. You will work within a trauma-informed, strengths-based, and community-controlled model of care that promotes healing, connection, and wellbeing.
About the Opportunity
You will be responsible for:
- Delivering culturally appropriate aftercare services following suicide attempts or crises
- Supporting risk assessments, safety planning, and coordinated care
- Building strong referral pathways with health, mental health, community, and support services
- Participating in multidisciplinary case management and care planning
- Providing outreach and community-based support
- Maintaining accurate documentation and reporting in line with organisational requirements
What You’ll Bring
- Experience in case management, group work, or community health
- Knowledge (or willingness to learn) of suicide prevention strategies
- Strong interpersonal, communication, and advocacy skills
- Ability to work independently and within a multidisciplinary team
- Commitment to Aboriginal self-determination and culturally safe practice
Essential Requirements
- Ability to build and maintain trust with Aboriginal community members
- Working with Vulnerable People Card
- National Police Check
- Valid driver’s licence
- COVID-19 vaccination record
