The TAC is an approved Aged Care Service Provider for Home Care Packages.
The program supports older people with complex care needs to live independently in their own homes.
We develop individual care packages to suit our clients needs and goals.
A variety of support measures which may be provided by our team include:
- Help with household tasks such as domestic assistance, shopping, and transport.
- Getting equipment to help clients, such as walking frames.
- Organising minor modifications in the home.
- Personal care support.
- Connecting the client to clinical care support such as nursing, allied health and physiotherapy services.
There are four levels of Home Care Packages starting at Level 1 for basic care needs to Level 4 for greater care needs.
